By accessing or using FrontaHealth's services, you agree to be bound by these Terms of Service. If you do not agree to these terms, you may not use our services. These terms apply to all users, including healthcare providers, administrators, and staff members.

2. Service Description

FrontaHealth provides AI-powered automation for insurance verification and appointment intake. Our services include but are not limited to:

• Automated insurance eligibility verification
• AI-powered appointment scheduling and patient intake
• Integration with practice management systems
• Customer support and training

3. User Responsibilities

As a user of FrontaHealth, you agree to:

• Provide accurate and complete information
• Maintain the security and confidentiality of your account credentials
• Use the service only for lawful purposes and in compliance with HIPAA regulations
• Not attempt to reverse engineer, modify, or disrupt our services
• Promptly notify us of any unauthorized access or security breaches
• Ensure your staff receives appropriate training on our platform

4. Free Trial and Subscription

FrontaHealth offers a 7-day free trial with no credit card required. After the trial period, continued use requires an active paid subscription. Subscription fees are billed monthly or annually based on your selected plan. You may cancel at any time with 30 days notice.

5. Payment Terms

• Fees are due monthly or annually in advance based on your plan
• All fees are non-refundable except as required by law
• We reserve the right to modify pricing with 30 days advance notice
• Failure to pay may result in service suspension or termination
• Setup fees, if applicable, are one-time charges

6. Data and Privacy

FrontaHealth acts as a Business Associate under HIPAA. All Protected Health Information (PHI) is handled in accordance with our Privacy Policy and Business Associate Agreement. You retain ownership of your data, and we will not use it except as necessary to provide our services or as required by law.

7. Service Availability

We strive to maintain 99.9% uptime but do not guarantee uninterrupted service. We may perform scheduled maintenance with advance notice. We are not liable for service interruptions caused by factors beyond our reasonable control.

8. Intellectual Property

All content, features, and functionality of FrontaHealth are owned by us and protected by copyright, trademark, and other intellectual property laws. You may not copy, modify, distribute, or create derivative works without our express written permission.

9. Limitation of Liability

FrontaHealth is provided "as is" without warranties of any kind. We are not liable for any indirect, incidental, special, or consequential damages arising from your use of our services. Our total liability shall not exceed the fees paid by you in the 12 months preceding the claim.

10. AI-Generated Content

Our AI systems generate responses and perform tasks autonomously. While we implement quality controls, you acknowledge that AI-generated content should be reviewed by qualified personnel before being used in patient care decisions. You remain responsible for all clinical and business decisions.

11. Termination

Either party may terminate this agreement with 30 days written notice. We may suspend or terminate your access immediately for breach of these terms, non-payment, or illegal activity. Upon termination, we will return or securely delete your data as specified in our BAA.

12. Modifications

We reserve the right to modify these terms at any time. Material changes will be communicated via email or through our platform at least 30 days in advance. Continued use after changes constitutes acceptance of the modified terms.

13. Governing Law

These terms are governed by the laws of the United States and applicable state laws. Any disputes shall be resolved through binding arbitration in accordance with the American Arbitration Association rules.
